What do patients want ? According to a Harvard study, as reported by Dave deBronkart on page 214 of Laugh, Sing, Eat Like a Pig, patients want the following. They are, in 2010, not really revolutionary. Why, as regards, our "Health!", are they still mostly only wishes?
- Availability of doctor or healthcare professional (informed of their case) by email and cellphone 24/7
- Access to their medical records via the Internet
- Offering newly diagnosed patients crash courses on their disorders
- Training and supporting patients to practice self-managed care
- Checking clinical practices against recommended medical guidelines
- Asking patients to critique services and suggest improvements (ex. PatientsOpinion.org)
- Involving patients in the governance of the clinic (public hospitals in France)
- Providing online support communities for patients
This is very useful and interesting indeed. But I would add one more item to the list, aiming to support those elderly who are not so capable at managing their own situation at home:
Automatic messages to carer or family carer if accidents happen or are about to happen, for example falls indoor and outdoor, smoke detection, extreme heat or cold, forgotten cooker, water flooding. This could be achieved with planning of smart home applications in all new homes

I would add another item onto this excellent list.
All providers should accept patients as part of their own caregiving team and respect and support their input and their desire to be an active participant in their own care.
